Please complete the necessary forms before your appointment. It is very important that we have all the requested information. If you need assistance filling out any of the forms, please arrive at least 30 minutes before the time of your scheduled appointment.
In order to make sure that all clients are seen in a timely manner and receive the most effective care, if you arrive at your exact appointment time and have incomplete paperwork, you may be asked to reschedule your appointment for another day or time.

If you are completing paperwork for your child(ren), all forms must be signed and dated by you and your child. If you share custody of your child(ren), you will need to bring the appropriate paperwork recognizing that you have the right to have your child(ren) participate in therapy services.
